Massage & Bodywork Consent and Service Terms
Plain-language terms for professional Massage & Bodywork services at InnerVital Wabash Chicago Loop or a participating clinic that accepts your request.
Nature and purpose of service
Massage/bodywork may involve professional hands-on or practitioner-guided techniques intended to support comfort, relaxation, mobility, and related wellness or care goals within the practitioner’s lawful scope. Services may include Licensed Massage Therapy, Tuina, Shiatsu, or Medical Qigong according to practitioner qualifications and patient needs.
Massage/bodywork does not replace medical diagnosis or treatment that you may need from a physician or another licensed healthcare professional. No particular outcome is guaranteed.
Your health information
You agree to provide accurate information about health conditions, injuries, surgeries, pregnancy, relevant medications, physician restrictions, and other circumstances that may affect massage/bodywork. Tell your practitioner about changes before or during a session.
Practitioner discretion
For safety and lawful scope, the practitioner may modify techniques, avoid areas, stop or postpone a session, request additional information, or recommend medical evaluation or clearance. If the practice determines before substantive service begins that the session should not proceed for clinical or safety reasons, an applicable session payment will be refunded or credited under the clinic’s approved policy.
Your control and withdrawal of consent
You may request less or more pressure, ask that an area not be touched, change positioning, request additional draping, pause the service, or end the session at any time. You may withdraw consent at any time.
Professional boundaries and draping
InnerVital Massage & Bodywork is a professional healthcare/wellness service. Sexual behavior, sexual requests, harassment, and inappropriate conduct are not permitted. The practitioner may immediately terminate a session if professional boundaries are violated.
Professional draping practices will be used where applicable, with only the area being worked on exposed as appropriate for the technique and your comfort.
Late arrival, cancellation, and rescheduling
Appointment extension time may not be possible. Late arrival may reduce the available hands-on session time so the next patient’s appointment is not delayed.
- 24 hours or more before appointment: Cancel or reschedule without a late-cancellation fee.
- Less than 24 hours: A late-cancellation fee of up to 50% of the scheduled session price may apply, subject to reasonable emergency exceptions.
- No-show: The scheduled session fee may be nonrefundable / charged, subject to reasonable emergency exceptions.
- Clinic cancellation: If the clinic cancels the appointment, the patient may reschedule without charge or receive a refund of any amount actually paid.
- Duplicate charge: Duplicate charges are refunded.
- Clinical contraindication before service: If the professional practice determines before substantive service begins that the session should not proceed for clinical/safety reasons, any applicable session payment should be refunded or credited according to the clinic's approved policy.
These service-specific terms supplement the site’s Refund & Cancellation Policy. The professional practice named on your appointment or receipt administers clinical cancellations and refunds.
First-session offer and payment
Massage & Bodywork in-person treatment is $160 at treatment, $144 pay now (save 10%), or 2 Treatment Credits. Payment is handled by the selected professional practice.
The Wabash offer is provided by InnerVital Wabash PLLC, is separate from InnerVital Membership, has no cash value, applies only to the eligible first-session offer, and cannot be combined with other promotional discounts unless the clinic explicitly permits it. For participating clinics outside Wabash, the clinic, practitioner, availability, price, merchant, and applicable terms will be confirmed before your visit.
Consent record
The protected intake records your three required attestations, patient name, and acceptance date and time. A typed name and clickwrap attestations are used; the experience does not present an invented handwritten signature.
Contact InnerVital before completing the intake. Do not use the intake for a medical emergency; call 911 or seek immediate medical care.
